Evaluation & Assessment
When we begin working with older adult residents, we promptly perform a comprehensive evaluation to assess mental, physical and nutritional health. This allows the clinical team to develop a comprehensive list of problems, strengths and weaknesses for the resident and determine the appropriate level of care for their physical, emotional and cognitive abilities.
Once we’ve established areas of growth for residents, our clinical team puts together personalized treatment plans. We utilize a mix of therapy modalities and interventions, including individual, group and family therapy. Our goal is to help residents develop effective coping strategies to reduce the recurrence of their disorder and help prevent relapse. We teach them exercises and skills to maintain or improve their cognitive skills with emphasis on utilizing them in daily living. This can lead to improved self-esteem and a greater sense of achievement.
Aftercare & Review
Our clinical team will work closely with facility staff on education and training to ensure that care is ongoing and aftercare planning for the residents is established. We also provide periodic, systematic reviews of our program’s performance by working with the resident, family and staff to check-in on resident improvement and progress. Our primary goal for our partner facilities is to enhance the quality of life for residents and increase the satisfaction of family members.
